Every young person who comes to us has a different story. Our process is designed to meet them where they are — not where the system expects them to be.

1. Intake Phase (8 weeks)

For: All new participants

Purpose: We start with a free 15-minute call — no forms, no pressure. If we're a good fit, we complete a thorough intake assessment covering your child's educational history, wellbeing, and goals. Most families tell us this is the first time someone actually asked the right questions.

What's Included:

  • 2 × 60-minute small group sessions per week (max 5 students)
  • 1 × 30-minute individual check-in session per week
  • Weekly parent/carer communication
  • 8-week functional capacity assessment report
  • Pathway recommendation report

Example Goal:Participant will increase social tolerance from 0 to 60 minutes in structured group setting over 8 weeks

Pricing:

  • Private Pay: $400/month ($100/week)

We provide a comprehensive report recommending which pathway is best for your child's needs and goals.

2. Stability Pathway

For: Young people who need ongoing stability and social connection

Purpose: Prevent isolation, maintain gains, build capacity through interest-based learning

What's Included:

  • 2 × 60-minute group sessions per week (interest-based projects)
  • 1 × 30-minute individual session per week
  • Fortnightly parent/carer communication
  • Quarterly progress reports

NDIS Alignment:

Example Goal:Participant will maintain social participation in community group 2× per week

Pricing:

  • Private Pay: $400/month ($100/week)
  • NDIS Rate: $180/week (claimable)

Who Is This For?

Young people who aren't ready to return to school/TAFE, but need ongoing support to prevent social isolation and maintain functional capacity.

Right for young people who need consistency before they can grow.

3. Bridge Pathway

For: Young people returning to school/TAFE who need ongoing support

Purpose: Supported re-entry with active liaison and relapse prevention

What's Included:

  • 1 × 60-minute “Reflect & Regulate” session per week
  • School/TAFE liaison and advocacy (as needed)
  • Fortnightly parent/carer communication
  • Monthly progress reports

NDIS Alignment:

Example Goal:Participant will successfully generalize regulation skills from Continuum environment to mainstream education setting

Pricing:

  • Private Pay: $250/month
  • NDIS Rate: $120/week (claimable)

Most young people who return to school without ongoing support experience relapse. The Bridge Pathway provides the safety net they need during this high-risk transition period.

4. Independence Pathway

For: Young people transitioning to home education, distance education, or vocational pathways

Purpose: Pathway planning, decision-making support, and transition documentation

What's Included:

  • Pathway exploration and decision-making sessions (4-8 weeks)
  • Transition documentation and portfolio development
  • Administrative support (enrollment, registration, etc.)
  • Final progress report and learning profile

Example Goal:Participant will successfully transition from re-engagement program to sustainable educational pathway

Pricing:

  • Private Pay: $600 (one-off)
  • NDIS Rate: Claimable under CB Improved Life Choices
